EvokeEHR is a bi-directional EHR that communicates and is integrated with the intelligence-driven Evoke360 platform. DataLink Software connects HIEs, EHRs, EDIs, social databases, and other sources across the country to give the physician insight into the necessary steps in the care path—assisting in closing Healthcare Effectiveness Data and Information Set (HEDIS®) gaps, driving preventative steps for patients, and supplemental benefits that may increase care path adherence by illuminating high-value interventions or available support services.

According to Brian, when EvokeEHR and Evoke360 “talk” with one another, medical assistants, SNFs, primary care physicians, or referral coordinators can see the complete health status of the patient and what may be impeding their care journey. “Within the platform, we show the member’s best pharmacy benefits via our pharmacy module and illuminate the top-performing specialists in the event additional care is needed,” explains Brian. In the pre-visit and post-visit modules, EvokeEHR highlights HEDIS gaps, chronic conditions that have not been completely assessed, or prescriptions that have not been filled. There is also a “linked” scorecard that quickly and easily shows urgent items the clinician needs to discuss with the patient.

To further illuminate the value of EvokeEHR, DataLink Software recently added a few customizations—pre-visit prep screens for quality and clinical prioritization and post-visit elements that ensure clinical documentation improvement (CDI). These pre-visit and post-visit steps improve clinical coding specificity and accuracy by 12% during the first year of utilization and improve chronic care management metrics and HEDIS measurement closure by 17%. DataLink Software’s mobile app and Chrome extension ensure higher adoption rates and impact by showcasing lab, pharmacy, and other physician’s evaluation findings.
